The patient has received intravascular contrast. Look at this movie frame by frame. Some structure is pretty unusual. The answer will be revealed when you click on the drop-down arrow.
This is a case of horseshoe kidney. The kidney looks bright because the images were taken while it was
filled with intravascular contrast. Although the renal pelves and ureters are often dilated in patients with
horseshoe kidneys, the attending radiologist thought the dilatation was more extreme in this case because
the patient had a markedly distended urinary bladder as a consequence of difficulty urinating.
